Inside the Surrogate Screening Process: What to Expect and How to Prepare

Becoming a surrogate is a deeply meaningful commitment, one that begins well before pregnancy itself. Before you’re officially matched, you’ll go through a thoughtful screening process designed to make sure you’re physically, emotionally, and practically ready for the journey ahead. This step plays an important role in protecting everyone’s safety and well-being, including supporting your health, safeguarding intended parents, and creating the healthiest possible start for the baby. Here’s what the screening process typically involves and how you can prepare with confidence.

1. Application & Background Checks

Before any screening begins, you’ll fill out an application that gathers your health history, lifestyle, and pregnancy background. This information, along with a standard background check, helps confirm eligibility and supports a safe, secure process for you and the intended parents.

2. Medical Records Review

Our team will request and review your medical records to ensure a surrogacy pregnancy wouldn’t pose any risks to your health.  

3. Interview with the Agency

Once your application and initial records are reviewed, you’ll meet with the agency for a one-on-one interview. This conversation gives the team a chance to get to know you beyond paperwork and ensures you have a clear understanding of what the surrogacy process involves, including expectations, timelines, and compensation. It’s also your opportunity to ask questions, share what you’re looking for in a match, and talk through any concerns. 

4. Psychological Screening

As part of screening, you’ll also meet with a licensed mental health professional. This conversation creates space to talk through your motivations, expectations, and any emotional or mental health concerns that could impact your surrogacy journey.

5. In-Person Medical Evaluation 

Once you are matched with intended parents, you’ll undergo a medical evaluation at the fertility clinic you’ll be using for IVF. Your provider will typically conduct a physical exam, collect bloodwork, and perform an ultrasound of the uterus. This step verifies that your reproductive system is healthy and ready to support implantation and pregnancy. 

Timing & Communication 

The full screening process can take several weeks — often 4–8 — depending on appointment availability, testing schedules, and any follow-ups needed. During this time, expect regular updates from our agency. We’ll guide you through appointments, results and your next steps after each requirement is complete. Occasionally, additional tests or consultations may be requested to ensure everything is thoroughly reviewed before final approval.
